Chest Surgery for Gender Affirmation

Chest surgery is often the first gender-affirming surgical step for transgender and non-binary individuals seeking to align their physical appearance with their gender identity. The goals and techniques differ depending on whether the individual is seeking a more masculine or feminine chest.

For those undergoing female-to-male (FTM) or female-to-non-binary transition, the primary goal is to remove chest tissue and create a flatter, more masculine chest contour. In some cases, liposuction alone may be sufficient, but most people also require removal of excess skin and repositioning or resizing of the nipples to try to achieve a more natural-looking result. The focus is on creating a chest appearance that minimises visible scarring and maximises gender congruence.

For those seeking chest feminisation (male-to-female or transfeminine), breast augmentation is the most common approach. This typically involves placing a breast implant beneath the existing chest tissue to create a fuller, more traditionally feminine breast shape. In some cases, a temporary tissue expander may be used first to gently stretch the skin before replacing it with a permanent implant at a later date. This approach is similar to breast reconstruction following mastectomy for breast cancer.

Dr Aggarwal is a leading surgeon in the field of breast reconstruction and reduction surgery in Sydney. His extensive experience with these procedures has provided him with the technical expertise and refined aesthetic judgment needed to perform gender-affirming chest surgery with exceptional results. His background ensures that patients receive a safe, technically precise, and natural-looking outcome tailored to their unique needs and goals.

The specific surgical techniques and options will be discussed in detail at your consultation. Dr Aggarwal performs gender-affirming chest surgery under general anaesthesia at accredited Sydney hospitals. Most patients require an overnight hospital stay.

Recovery generally takes about 4–8 weeks, depending on the extent of surgery and the desired results. While complications are rare, they can include infection, fluid collection (seroma), delayed wound healing, scarring, numbness in the skin or nipples, and asymmetry. For breast implants, there are potential long-term issues such as implant rupture or capsular contracture (scar tissue around the implant), which may require further surgery. For more information about implant-related procedures, please see more information on implant revision here.

A comprehensive discussion of risks and benefits will be provided during your consultation with Dr Aggarwal. In Australia, gender-affirming chest surgery – including gender reassignment surgery in Sydney – may be partially covered by Medicare if it meets certain medical criteria, and private health insurance may also contribute to the cost.

We recommend contacting your health fund and Medicare to check your cover and eligibility. We are happy to assist you with any paperwork required to obtain coverage for your gender-affirming chest surgery.
